In recent years, Americans have faced record levels of obesity, diabetes, and other chronic diseases—and the numbers are not improving as they should.
At the same time, both employers and employees continue to struggle with skyrocketing health care costs. One reason costs are so high, according to recent studies, is a lack of clear communication between doctors and patients.
In response to this crisis, Dr. John Wasson, a researcher and professor at Dartmouth Medical School, developed the "How's Your Health?" program to help Americans improve their health by fostering better communication with their medical providers.
How's Your Health, NJ? is a unique initiative sponsored by NJHCQI with the New Jersey State Chamber of Commerce and the State of New Jersey. The State of New Jersey recently adopted How's Your Health, NJ? for use in its county "One-Stop Career Centers."
New Jersey residents can log on to www.njhcqi.org and take a free, anonymous, confidential, and user-friendly ten-minute survey. There is no advertising on the site and no personal identifiers (cookies) are collected. Employers who promote the use of the survey with their workers can collect aggregate data to help them tailor wellness programs in the workplace.
Upon completion of the survey, each respondent immediately receives a customized education packet on preventive care and chronic illness management, as well as a list of important questions designed to help boost effective communication with doctors and other health care providers.
